Correction of Inadequate Angulation of External Hexagon Implants using a Lateral Tube: Guarantee of Reversibility

Abstract

This study aimed to report the case of oral rehabilitation on implants, emphasizing the use of additional elements of retention type "screw tube" (micro-UCLA) to correct unwanted angulations of implants. Patient SBS, female, attended the dental prosthesis clinic at FOA/UNESP with the main complaint of lack of aesthetics and function of the RPD and report of having been consulted for implant installation surgery approximately 6 years ago. The clinical examination revealed the absence of elements 12-22, and the radiographic examination showed the presence of 3 HE implants installed in regions 12, 21 and 22. Rehabilitation of the region was proposed with a prosthesis on splinted implants due to the pontic that would be made. With the patient's consent, the implants were reopened, and impressions were taken to make the temporary ones, where non-ideal angulation of implants 12 and 22, slightly buccal, was verified, which indicated the use of prosthesis with angulated intermediates, or with dynamic UCLA, or cemented for correction. Thus, to maximize aesthetics and give the treatment a reversible character, the creation of an FPP cemented on an implant with an association of 2 micro-UCLAS per lingual was proposed. The treatment was completed satisfactorily, and the patient was quite satisfied with the result. Despite making the treatment slightly more expensive, the system worked satisfactorily and promoted the possibility of future removal of the prosthesis, in case there was a need to retighten the screws of the trunnions connected to the implants.
