Use of High-Intensity Laser as an Alternative to Conventional Surgery for Gingival Depigmentation: A Systematic Review

Objective: To evaluate the current scientific evidence on the results of high-intensity laser compared to conventional techniques for correcting gingival hyperpigmentation. Methods: A systematic review following the PRISMA checklist. The systematic review protocol was registered in the PROSPERO CRD42020173752 database. Seven electronic databases were accessed as primary sources of study. "Gray literature" was also included to avoid selection and publication bias. The risk of bias was evaluated using the Joanna Briggs Institute's Critical Assessment Tool for Systematic Reviews. Results: The lasers used in the studies were laser diode; Er, Cr laser: YSGG; Er: YAG laser and Nd: YAG laser, the diode laser being the most tested. When comparing the diode laser with scraping using a scalpel, the use of the laser showed little or no bleeding during treatment, less pain during and after surgery, absent or mild pain in the postoperative period, slightly longer healing and a more accepted procedure by the patient. Comparing the Nd: YAG laser and scalpel the results were similar to the diode laser. Conclusion: The use of high-intensity laser has satisfactory clinical results and is a safe alternative to surgical treatment with a scalpel blade for gingival depigmentation. However, the results need to be analyzed with caution, due to the risk of moderate or high bias in most of the eligible studies and the heterogeneity in relation to the tested protocols.
