Ludwig's angina is a rapidly spreading cellulitis, which mainly affects the floor of the mouth, progressing towards the submandibular, sublingual, and submental spaces, which can cause airway collapse, resulting in ventilatory failure and death. This paper aims to describe a clinical case report of Ludwig's Angina of odontogenic origin who underwent surgery at Hospital Regional Tibério Nunes in Floriano-PI, with emphasis on airway management. A 23-year-old female patient without comorbidities was seen by a dentist three times, with toothache before admission due to Ludwig's Angina. After the patient was admitted to the Hospital Regional Tibério Nunes by the Bucomaxillofacial Surgery and Traumatology team, it was discovered that she had cellulitis with airway obstruction. She needed an emergency surgical debridement of the airway and extraction of the infectious site. Early recognition and rapid intervention are critical in the treatment of this serious condition, as airway management in patients with Ludwig's angina remains challenging. The choice of the safest technique must be based on the clinical picture, the available technical condition, and the urgent need to preserve the patient's life.
