Management of Open Fractures due to Trauma: Literature Review

Introduction: Despite the advances made in the treatment of fractures and in the prevention of infections, open fractures remain a therapeutic challenge with varying levels of evidence to support some of the most used practices. Objective: To synthesize therapeutic approaches for exposed fractures caused by trauma based on studies published in databases. Methods: literature review in the MEDLINE, PUBMED, LILACS and SciELO databases, of articles from studies developed on this subject, published between 2015 and 2019, in English, Portuguese or Spanish. In the end, 10 articles were included. Results: nine articles addressed antibiotic prophylaxis and the antimicrobial class used, duration and spectrum of action. Five studies addressed the time until debridement and external fixation of the fracture according to the degree of injury. Conclusion: it is concluded that the antibiotic should be started prophylactically early, preferring the use of cephalosporins, with variable time according to the degree of exposure. The ideal time for debridement should not exceed 48 hours after the injury, preferably within 6 hours.
