Abstract

The aim was to conduct a systematic review to determine the influence of the type of direct intraradicular posts on the resistance of endodontically treated permanent single-rooted teeth with wide canals. This systematic review followed PRISMA criteria for reporting systematic reviews and meta-analyses. The research question was formulated using the P.I.C.O. strategy. The study selection process was conducted by two independent reviewers, using the same pre-trained eligibility criteria. The selection was carried out in four stages: title, abstracts, full text, and quality analysis. Data were analyzed using Stata11.0 software. Among the treatments for single-rooted teeth with wide or weakened canals, restorations with fiberglass posts combined with composite resins, accessory fiber posts, or anatomical posts demonstrated higher fracture resistance and bond strength than those treatments without reinforcements, which used posts with resin cements, flowable resins, or resin reinforcement tapes, and were not different from the positive control of cast metal posts. The most common fractures were repairable or non-catastrophic. The control groups with cast metal posts showed the highest fracture resistance results, and groups with regular diameter canals demonstrated better bond strength and fracture resistance than teeth with wide canals. The use of remodeled fiberglass posts, reinforced with composite resins or complemented with accessory posts, as a restorative treatment for single-rooted teeth with wide endodontically treated canals, presents adequate fracture resistance and bond strength and a favorable fracture pattern.
